Policy For
ARRANGEMENT
FOR HEALTH CARE TRANSPORTATION
Policy: All
residents will be provided transportation to and from
medical offices with 20 miles. The first two trips within
each month shall be free; thereafter, the resident will be
charged $20.00 per trip. If the resident is unable to be
transported in the facility vehicle, the facility will
arrange for a special transport, (i.e. LaCross or
Ambitrans), at the resident's request. If the resident
needs an escort to appointments. the resident will be
charged $20.00 per escorted appointment. The resident (or
responsible party) is responsible for payment of all
charges for special transportation. This is also addresses
in the Contract for Care.
Procedure:

The resident
should notify the Director of Resident Care, or the secretary
in the administration office that they need to
schedule a medical appointment, or; the director of
Resident Care will proceed to notify and make
appointment with the resident's physician if there is
a significant change in the resident's health status.
(The resident's family will be notified of the
situation and informed of the appointment.)

An appointment
will be made with the resident's health care provider
and the appointment will be placed on the
transportation log.
